Tirzepatide is a synthetic dual GIP/GLP-1 receptor agonist developed by Eli Lilly: a 39-amino-acid peptide engineered from the native GIP sequence with a fatty-diacid moiety for albumin binding. The FDA approved it as Mounjaro for type 2 diabetes on 13 May 2022 and as Zepbound for chronic weight management on 8 November 2023; Mounjaro is also authorised by the EMA.

The SURPASS trial programme characterised its dual-receptor mechanism and glucose-lowering action in adults with type 2 diabetes, including SURPASS-1 (Rosenstock et al., 2021) and a head-to-head comparison with semaglutide (Frías et al., 2021). The SURMOUNT programme (Jastreboff et al., 2022) studied its effect on body weight in adults with obesity or overweight through the same mechanism.

As Mounjaro and Zepbound, tirzepatide is prescription-only, manufactured and reviewed under FDA and EMA oversight, and its US prescribing information carries a boxed warning for thyroid C-cell tumours observed in rats. Research-use-only tirzepatide vials sold outside that supply chain are not those products and have not been reviewed for safety, effectiveness or quality. The FDA has warned of fraudulently compounded tirzepatide and has warned companies that illegally sold unapproved tirzepatide falsely labelled ‘for research purposes’.
